• Case Managers will coordinate services across the continuum of care in various capacities, such as with social, emotional, physical and financial support services.
  • They will make referrals and provide expertise regarding community social-service agencies.
  • Case Managers will also assist the consumer in identifying barriers to care and develop a specific integrated plan of care in collaboration with the consumer, family, and provider, and conduct face-to-face consumer assessments by visiting the consumer in his/her community, place of residence, or facility.
