Case Managers will coordinate services across the continuum of care in various capacities, such as with social, emotional, physical and financial support services.
They will make referrals and provide expertise regarding community social-service agencies.
Case Managers will also assist the consumer in identifying barriers to care and develop a specific integrated plan of care in collaboration with the consumer, family, and provider, and conduct face-to-face consumer assessments by visiting the consumer in his/her community, place of residence, or facility.
