OUR SERVICES

Our case managers facilitate and coordinate services across the continuum of care. We assist our clients with health and mental health issues, housing and food insecurity, transportation needs, and much more. Case management responsibilities include:

  • Conducting comprehensive assessments to identify the services needed.
  • Creating and implementing care plans based on comprehensive assessments.
  • Making referrals to community service organizations, medical professionals, and government agencies.
  • Evaluating and monitoring progress.
  • Collaborating with health and mental service providers.
  • Assisting clients in identifying barriers to care.
  • Providing services that are culturally sensitive and competent.
